According to a 2023 World Health Organization report, climate-induced health emergencies inflict massive financial losses on national economies annually. Shifting toward ecological prevention is a vital strategy for protecting human lives.
Fragmented government mandates prevent effective responses to complex environmental health threats. The Department of Environment and Natural Resources (DENR) focuses exclusively on land conservation. The Department of Health (DOH) concentrates on hospital management and medical treatment. The Office of Civil Defense (OCD) and the Department of Social Welfare and Development (DSWD) handle emergency disaster operations separately. Uniting isolated agency operations enables cohesive action for public safety.
Nature-based solutions restore natural ecosystems to protect public health and build climate resilience. Mangrove coastal buffers absorb heavy storm surges and prevent severe inland flooding. Urban green canopies cool dense cities and filter harmful air pollutants. A 2022 United Nations Environment Programme study confirms green infrastructure as the most cost-effective disaster risk reduction method. Healthy natural habitats directly create healthier human populations.
Western Visayas experiences severe climate risks requiring immediate ecological intervention. A 2025 World Wildlife Fund report highlights ecosystem restoration as essential for protecting regional infrastructure and road networks. Restoring degraded national forest lands delivers direct climate adaptation benefits to millions of citizens. Integrating ecological restoration into regional development plans prevents massive economic losses while protecting coastal communities.
Iloilo Province leads regional ecological restoration through integrated development planning. The provincial government launched the MoRProGRes initiative to attain official Forest Province status by the year 2044. Governor Arthur Defensor Jr. and Dr. Raul Banias, Provincial Administrator, established the Tanum Iloilo program to drive community tree planting across upland and coastal zones. Data from the Provincial Government Environment and Natural Resources Office (PGENRO) in October 2024 shows local communities planted over 8.3 million tree seedlings. A 44 percent seedling survival rate demonstrates strong community commitment to land recovery. Forest cover in Iloilo dropped to 12 percent over past decades, making continuous tree growing critical for water security and biodiversity protection.
Environmental planning in Iloilo centers on protecting the Tigum-Aganan Watershed. Originating in the central mountain ranges and flowing into the coastal plains of Iloilo City, the Tigum-Aganan river system sustains the water supply for the city and seven surrounding municipalities. Delivering 75 million liters of clean water daily across 297 barangays, the watershed meets the drinking and sanitation needs of roughly 1 million residents (MPIW). Beyond residential use, this reliable freshwater supply powers key commercial centers, industrial parks, universities, hotels, and medical facilities throughout Metro Iloilo.
Reforestation directly restores river stability and secures regional freshwater supplies. Tree loss in upper watershed zones causes severe soil erosion and intensifies river flooding during typhoons. Reforestation under the Tanum Iloilo program stabilizes riverbanks and recharges underground water reserves. Tree root networks absorb floodwaters and naturally filter sediment, lowering municipal water processing costs. Maintaining dense watershed forests guarantees continuous water availability during severe heatwaves.
Expanding agroforestry projects around watershed protection areas strengthens local food production and eco-tourism. Upland farming families cultivate fruit-bearing trees alongside native species and bamboos to generate income while protecting soil health. Tree growing activities across municipal plazas and private properties connect rural forest conservation directly with urban water consumers in Iloilo City. Implementing a ridge-to-reef management model secures clean land, air, and water for future generations.
Iloilo City complements provincial environmental goals through successful urban ecosystem restoration. The United Nations designated Iloilo City as a Generation Restoration Role Model City for the 2021 to 2030 decade. Local communities planted over 100,000 mangrove saplings along the Iloilo River since 2010, restoring 70 hectares of coastal wetland ((ECCP/cabidigitallibrary.org). Relocating 5,000 informal settler families to safer housing sites reduced river pollution and restored native fish species. The Iloilo City Comprehensive Land Use Plan institutionalizes nature-based solutions for urban sustainability.
Joint programs between Iloilo Province and Iloilo City maximize ecological and disaster protection benefits. Local government units, national agencies and corporate partners planted thousands mangrove saplings along Sunset Boulevard in March 2025. The Provincial Disaster Risk Reduction and Management Office (PDRRMO) supports eco-friendly tree parks to expand community green space. The collaborative Bakhawanihan Project mobilizes municipal leaders and volunteer groups for coastal cleanups, transforming degraded shorelines into natural storm barriers.
Thriving natural ecosystems and green infrastructure produce immediate, measurable reductions in human disease and hospital admissions worldwide. A global study tracking 11.4 million hospital admissions reveals that surrounding urban greenness reduces mental disorder hospitalizations by 7 percent, preventing 7,712 hospital admissions annually (The BMJ, 2025). Higher residential green space coverage also lowers hospital admission risks for circulatory and cardiovascular diseases by 17 percent while reducing pollution-driven heart attack admissions by 10.4 percent (Environmental Research, 2024). Furthermore, dense vegetation filters fine particulate matter and nitrogen dioxide to prevent a 30 to 45 percent increase in respiratory hospitalizations among populations lacking natural green buffers (European Respiratory Society, 2024). Growing up with abundant natural green space cuts the long-term risk of developing psychiatric disorders by 15 to 55 percent during adolescence and adulthood (Proceedings of the National Academy of Sciences, 2019). Active ecosystem preservation across urban canopies and coastal wetlands directly mitigates heat mortality, non-communicable diseases, and waterborne infection outbreaks (World Health Organization and The Nature Conservancy, 2025).
National health policy must recognize protected ecosystems as critical public health infrastructure. Updating Philippine Agenda 21 connects biodiversity conservation with disease prevention targets under global Sustainable Development Goals. The Department of Health (DOH) needs to incorporate nature-based strategies into long-term medical development plans, such as constructing hospitals with therapeutic green landscapes to improve patient recovery and lower building temperatures.
To strengthen primary health, the Philippine Institute of Traditional and Alternative Health Care (PITAHC), an attached agency under the DOH, was created by Republic Act 8423 (Traditional and Alternative Medicine Act of 1997) to institutionalize, research, and regulate traditional practices. PITAHC has broadened its recognition of therapies that complement modern medicine, including acupuncture, massage, herbal medicine, chiropractic, osteopathy, reflexology, naturopathy, and energy-based practices like Qigong and Reiki. Filipino traditions like Hilot (therapeutic massage), Ventosa (cupping therapy), and aromatherapy thrive under this framework.
Herbal medicine remains exceptionally accessible, with the DOH approving ten medicinal plants for common ailments. Scientific studies validate that Soursop (Guyabano) leaves and mulberry contain beneficial antioxidant compounds, Aloe vera reduces digestive inflammation, and bignay and insulin plant leaves help regulate blood sugar levels. Furthermore, the Food and Drug Administration (FDA) standardizes these plant medicines to ensure health security. Fast-tracking approval for certified herbal remedies reduces reliance on imported pharmaceuticals and ensures critical medical supplies remain available when severe typhoon disasters disrupt synthetic medicine supply chains.
Synchronized inter-agency coordination establishes a complete defense against climate-induced health crises. The DENR leads watershed and coastal conservation to safeguard clean water resources. The DOH builds green hospital infrastructure to reduce heat stress and enhance recovery. The FDA standardizes natural medicines, while the PITAHC validates traditional herbal therapies for public wellness.
Nature-based solutions offer a powerful dual approach: (1) mitigating climate change and (2) fostering preventive health and sustainability in daily community life. By moving away from reactive crisis management, nature-based solutions deliver major benefits. Restoring natural ecosystems like forests, mangroves, and wetlands creates a powerful dual defense that actively combats climate change while directly transforming public health. By pulling carbon dioxide from the atmosphere, lowering extreme urban heat during heatwaves, and absorbing heavy storm surges, green infrastructure shields communities from severe environmental hazards. Simultaneously, these thriving natural spaces empower healthier lifestyles by purifying the air to prevent asthma, naturally filtering watershed supplies to guard against waterborne illness, and preserving the native medicinal plants needed for affordable herbal care. Furthermore, accessible parks and restored riverbanks naturally encourage daily outdoor exercise and recreation, significantly lowering stress levels, obesity, and cardiovascular disease across entire populations.
By embedding ecosystem and biodiversity protection into the National Objectives for Health (2023–2028), the Department of Health (DOH) can build lasting resilience through green infrastructure, watershed protection, and proactive environmental disease surveillance. Achieving this vision requires cross-agency execution: formalizing joint administrative orders between the DENR and DOH will institutionalize ecosystem-based healthcare, transforming natural habitats into recognized assets for public health while aligning national strategy with the UN Sustainable Development Goals. Ultimately, this pioneering model positions the Philippines to demonstrate to the world that safeguarding nature is the most effective foundation for long-term public health and climate security.
